Membership Freeze Request

What you need to know about membership freezes based on your membership agreement: You may freeze your membership up to two months per year. Freezes must be in whole month increments and have a beginning and an end date. Your membership will automatically start back up at the end of your freeze. This form must be submitted 30 days in advance of the next membership cycle. A membership cycle begins on the first of each month. We cannot honor freeze dates with less than a 30-day notice or without this form. You may be billed one more membership cycle if this form is not submitted with enough notice.

For example if you want to freeze your membership for the month of August, your submission must be in by July 1st.
